Description of service: Community Participation Support is designed to complement and support community life alongside employment as a primary goal. This involves engagement in integrated community settings and activities alongside individuals without disabilities who are neither paid nor unpaid caregivers.
To effectively implement Community Participation Support Services within the Pennsylvania Office of Developmental Programs (ODP), our agency will focus on providing opportunities and support for community inclusion and fostering skills for competitive-integrated employment. Our services aim to facilitate active and valued participation in a wide range of integrated activities tailored to each participant’s interests, preferences, talents, and strengths, aligning with their desired outcomes related to employment and community involvement. Our approach will involve conducting comprehensive assessments, developing individualized service plans, and coordinating services in strict adherence to Pennsylvania ODP guidelines.
The primary goal of these services is to assist individuals with intellectual and developmental disabilities (IDD) in maintaining a clean, safe, and comfortable living environment by providing assistance with household tasks and chores.
Our implementation plan for In-Home and Community Support services in compliance with Pennsylvania Office of Developmental Programs (ODP) regulations focuses on providing personalized assistance and guidance to participants, fostering their independence and meaningful engagement in community life. We will ensure staff training and qualifications meet ODP standards, develop individualized service plans in compliance with Pennsylvania Department of Health’s Intellectual Disabilities Services ISP guidelines In-Home and Community Support is a direct service provided in home and community settings to assist participants in acquiring, maintaining, and improving the skills necessary to live in the community, to live more independently, and to participate meaningfully in community life. To the extent that In-Home and Community Support is provided in community settings, the settings must be inclusive rather than segregated. Services consist of assistance, support, and guidance (physical assistance, instruction, prompting, modeling, and reinforcement) in the general areas of self-care, health maintenance, decision making, home management, managing personal resources, communication, mobility and transportation, relationship development and socialization, personal adjustment, participating in community functions and activities, and use of community resources.
